Hemodynamic Monitoring and Fluid Management in Thoracic Surgery Requiring One-lung Ventilation Case Study
ثبت نشده
چکیده
INTRODUCTION Thoracic surgery frequently requires one-lung ventilation (OLV). Hemodynamic changes are significant during OLV and surgical manipulation and precise management of fluid administration is vital to postoperative recovery and surgical outcome. The FloTrac system, a minimally invasive hemodynamic monitoring device, with continuous cardiac output (CO) and stroke volume variation (SVV) is particularly useful in this patient population, especially during episodes of hemodynamic instability.

With the introduction of new techniques and advances in the thoracic surgery fields, challenges to the anesthesia techniques had became increasingly exponential. One of the great improvements that took place in the thoracic surgical field was the use of the robotically assisted thoracic surgical procedure and minimally invasive endoscopic thoracic surgery. One lung ventilation technique represe...
متن کاملGoal-Directed Fluid Therapy Using Stroke Volume Variation Does Not Result in Pulmonary Fluid Overload in Thoracic Surgery Requiring One-Lung Ventilation
Background. Goal-directed fluid therapy (GDT) guided by functional parameters of preload, such as stroke volume variation (SVV), seems to optimize hemodynamics and possibly improves clinical outcome. However, this strategy is believed to be rather fluid aggressive, and, furthermore, during surgery requiring thoracotomy, the ability of SVV to predict volume responsiveness has raised some controv...
متن کاملChanges in Blood Gases and Hemodynamic Parameters in Patients Undergoing Lung Resection Surgery and Its Clinical Implications
Introduction: Lung resection surgery is a challenge to thoracic surgeons. Outcome of surgery depends on patients’ tolerance to reduced lung volume and hemodynamic alterations. The present study aimed to investigate the changes in blood gases and hemodynamic parameters in patients undergoing lung resection surgery and the associated clinical implications. Materials and methods: This study includ...
متن کامل[Behaviour of stroke volume variation in hemodynamic stable patients during thoracic surgery with one-lung ventilation periods].
INTRODUCTION In last few years, emphasis was placed in goal-directed therapy in order to optimize patient's hemodynamic status and improve their prognosis. Parameters based on the interaction between heart and lungs have been questioned in situations like low tidal volume and open chest surgery. The goal of the study was to analyze the changes that one-lung ventilation can produce over stroke v...
متن کاملDiagnosis Accuracy of Mean Arterial Pressure Variation during a Lung Recruitment Maneuver to Predict Fluid Responsiveness in Thoracic Surgery with One-Lung Ventilation
Background. Lung recruitment maneuver (LRM) during thoracic surgery can reduce systemic venous return and resulting drop in systemic blood pressure depends on the patient's fluid status. We hypothesized that changes in systemic blood pressure during the transition in LRM from one-lung ventilation (OLV) to two-lung ventilation (TLV) may provide an index to predict fluid responsiveness. Methods. ...
